For a test-and-quarantine policy to be useful, you don’t have to pull every infected person out of circulation. Think of it as a race between how many infected people you pull out of circulation and how many people get infected by the folks who your test fails to catch. You come out ahead compared to doing nothing.
But a test can be bad the other way, easily producing one false positive for every true positive. You could easily end up quarantining one healthy person for every sick person. Of course, what we are doing now is at least as arbitrary.
Tyler Cowen points to a paper with a model (what else?) that supports doing testing and confining even when the tests are bad.
I’m sure it works well in the model. In the real world, I can think of a number of difficulties with execution. Show me a project management chart that includes all the steps needed before you can even start. Then with flawed tests, it takes much longer to get the benefits, and more costs are imposed on the false positives.

Another Alcorn pointer. They try to get beyond numerator analysis and estimate the infection fatality rates for different demographic groups. But their methods struck me as sketchy, so I am just quoting the raw data.

Here we report that 50% of the variance of ‘predicted covid-19’ phenotype is due to genetic factors. The current prevalence of ‘predicted covid-19’ is 2.9% of the population. Symptoms related to immune activation such as fever, delirium and fatigue have a heritability >35%. The symptom of anosmia, that we previously reported to be an important predictive symptom of covid-19, was also heritable at 48%. Symptomatic infection with SARS-CoV-2, rather than representing a purely stochastic event, is under host genetic influence to some extent and may reflect inter-individual variation in the host immune response. Viral infections typically lead to T cell activation with IL-1, IL-6 and TNF-α release causing flu-like symptoms such as fever. The genetic basis of this variability in response will provide important clues for therapeutics and lead to identification of groups at high risk of death, which is associated with a cytokine storm at 1-2 weeks after symptom onset

Roughly 99.9 percent of American firms, or 30 million, fit the definition of small business used by the Small Business Administration (SBA), and together these firms employ roughly 65 percent of American workers. The devastation of the small-business sector could therefore be disastrous for American families.
The stock market is relatively placid, and meanwhile a whole way of life seems about to go under for many people. That might not be sustainable.
I readily grant that if left to their own devices many individuals will make sub-optimal decisions, but primarily costing themselves and not others. I also believe that if individuals were left to their own devices we would not see anything close to what “re-opening the economy” sounds like. But giving decision-making power to President Trump and the various governors seems more obviously right to most other people than it does to me.

Put your own probabilities and multiply them: The resulting probability is very small. I asked some of my colleagues for their probabilities, and the product always came below 3 percent.
His high inflation scenario is one in which our government can no longer pay the bills except by printing money. That is actually a hyperinflation scenario, as I imagine Blanchard would agree. Even at a probability of less than 3 percent, Anti-fragile Arnold does not want to take those chances.
Regarding 1, my understanding is that the rate of false positives for a COVID-19 test should be rare if the proper protocol is followed. My guess is that there are still issues with protocol in some settings, especially with some of the more rapid tests (sometimes, but not always called point of care tests). I believe the rate of false positives for a test analyzed at a good lab is 1% or less (although admittedly that is just a guess). Interestingly, I could find little liturature on this subject, at least for COVID. But my hunch is, I don’t think we should discount the testing approach because of issues encountered with false positives, as I would think (hopefully?) that these will be solved soon.
On point 6, high inflation, you need to consider the greater likelihood of severe deflation than hyperinflation. Severe deflation is at least as damaging to society as hyperinflation, so Anti-fragile Arnold should prefer a policy that minimizes both extremes.
I like severe deflation. The less I have to pay for things, the better. I wish that health care technology prices fell as fast as other technologies.
The overdraft idea by DeRugy and Arnold is great, I hope we try it soon. Cuts right through the fog of the SBA and bank loans.
Maybe we should let individuals use overdrafts to pay their rent and mortgage, Otherwise we may see a hideous crisis of default and even homelessness.
Overdrafts are supposed to go to individuals, and they should use it to pay rent and mortgages, rather than arbitrarily breaking those contracts.

Ultimately, how? Maybe if we get the miracle vaccine in that time. But otherwise, limiting who becomes infected only prolongs the crisis. This virus isn’t going away. It is to prevalent and to transmissible. We build immunity in the population, we maybe get a vaccine that protects those most likely to have a bad “trip”. Then the virus slips into the background noise with the influenza variants, common cold viruses, etc.
Believing this virus can be killed out of the human virus pool is leading to very bad decisions. I see the allure, the prospect is very intellectually clean, but it isn’t reality.
The healthcare industry touts its “innovation” every chance it gets (typically in support of policy favorable to the industry). Is it any wonder we believe a cure is imminent?
I think the case of the USS Theodore Roosevelt is an instructive example that has gotten lost in the noise recently. This USA Today affiliate in Guam reported on Friday (April 24) that the Navy has now tested 100% of the nearly 5,000 sailors aboard, with 4,098 negative results, 840 positive results, and so far one death (which may have had nothing to do with the coronavirus), four current hospitalizations, and zero in the ICU. This stands in marked contrast to the numbers on the cruise ships, where people also lived in very close quarters, but where the typical guest was elderly and more likely to be obese, not young and fit like Navy sailors.
It’s also a far cry from the scenarios proposed by the mainstream media back when the USS Theodore Roosevelt became a story in late March: bad President Trump failed to protect those aboard, and now they will suffer mass casualties as a result. Most of the media have since all but stopped reporting on the encouraging and perhaps surprising robustness of these sailors, and now only drag out the story from time to time when it suits the narrative of bad President Trump wrongly punishing good Captain Crozier.
